Agency to Address Education & Hospital Readmission Penalties Under Affordable Care Act
SCOTTSDALE, AZ - February 27, 2014 -- Assisting Hands Home Care Scottsdale today announced that the agency is celebrating its five-year anniversary serving clients in the Scottsdale and surrounding valley area. Owner Terry Madden will expand the focus of her company to market her services toward hospital executives, case workers and discharge planners struggling to reduce the amount of readmission penalties their facilities are suffering under the Affordable Care Act.
Since the opening of Assisting Hands Home Care Scottsdale in 2009, the non-medical, in-home caregiving agency has enjoyed annual double-digit growth and a solid word-of-mouth referral reputation in the industry. Yet the availability of these services is widely unknown by possible consumers and by those in the medical industry.
"Education is key to be able to help seniors and those with chronic illness so that they can remain in their residence of choice as long as they can remain safe," said Terry Madden, owner and Certified Senior Advisor. "Even lesser known is how our services can be referred by hospital administrators to address the four key triggers that lead to a patient being readmitted within 30 days."
Upon discharge a patient is given a plan of care to be followed to avoid future hospital stays. Under rules of the Affordable Care Act, if a patient is readmitted within 30 days, a hospital must forfeit one percent of Medicare revenues and reimbursement for those patients. When these plans are not followed, the resulting penalties negatively affect a facility’s bottom line.
The four key "triggers" to readmission that Assisting Hands services, address are; lack of social support, mismanagement of medication, not understanding the disease process and missing followup doctors appointments.
"Unfortunately, some patients do not have a loved one or family member to care for them, helping them to follow the plan," said Madden. "We have the resources to ensure the plan is followed."
